How Does It Work?
A medication management review (MMR) is a comprehensive assessment aimed at optimising a patient's medication regimen. In this review, the referring clinician, who initiates the process due to concerns about the patient's treatment or medication adherence, collaborates with a Teladoc Health accredited pharmacist.
Once a referral is received, Teladoc Health will obtain the patient’s consent to proceed with the review and then coordinate the collection of medical records to support the review.
The pharmacist thoroughly evaluates the patient's current medications, including prescriptions, over-the-counter drugs, and supplements, to identify potential interactions, duplications, or gaps in therapy.
The patient actively participates in the review, providing insights into their medication usage, adherence challenges, and health goals. Additionally, support people chosen by the patient, such as family members or caregivers, may be included in the discussion to offer encouragement, help clarify the patient’s experience, and reinforce the recommendations made during the review.
The ultimate goal of the MMR is to enhance medication safety, improve health outcomes, and foster a collaborative approach to managing the patient’s health.
